1. All current insurance cards
2. Drivers License or other identification
3. Working email address
4. Phone number that accepts text messages and / or voicemail

The email and phone number are used for patient portal activation and communications. The patient portal allows faster access to patient / provider communications and rescheduling

For new patients there are several forms to review and one to complete the Patient Registration Packet. The forms can be accessed online by following the Patient Forms button below. The Patient Registration form can be downloaded, filled out, and brought to the office on first appointment
