Website Acess, Download and submitting
1. Click one of the forms in the list to view
2. For the Patient Registration Form
3. A pdf will appear
4. Fill in all require fields
5. Select Save and or Print
6. Add signature
7. See instructions below for submitting
Patient Portal Access
1. Login to Patient portal
2. Scoll down and select "View Documents"
3. Select "View" or "Download and a pdf will appear
4. Fill in all require fields if any
5. Select Save and or Print
6. Add signature if needed
7. Some documents may allow signatures within the patient portal
8. Select "Signature Requested" and follow on screen prompts
Submitting Patient forms
1. For non medical forms they can be Emailed, Faxed, or brought on
appointment day
2. For medical forms they can be Faxed, or brought on appointment
day
